| Claim Form Request
We recognise that if you are requesting a claim form from us then your travel arrangements or holiday have been cancelled, disrupted or spoilt due to illness or misfortune. We aim to facilitate the claims process as far as possible. In order to ensure we are able to deal with your claim as quickly and efficiently as possible we would ask that you take time to read and complete the claim form and send it to us with all the supporting documentation requested. Please do not forget to sign your form at the foot of the back page as failure to do so will delay your claim.
You can request a claim form in several ways. Before you do so, you will need to have available the following information:
- The date your policy was issued
- A policy number (if available)
- Name of insurance policy/purchase details
- Your departure date
- Type of claim you wish to make (e.g. cancellation, medical expenses/curtailment, baggage/money, travel delay)
- Contact us by e-mail at travelclaims@cl-uk.com (you must provide the information requested above in the e-mail) or
- Telephone us on 0845 604 9855 (between the hours of 9am and 5pm Monday to Friday, excluding Bank Holidays) or
- Click on the appropriate claim form request link on the left of this page
If you choose to complete your claim form on-line, this can be e-mailed back to us, which will enable us to start the assessment process. However, we will require a hard copy of a signed claim form, together with original supporting documentation before any payment can be made.
